  3.      There are a couple different ways you can go about contributing to 
  4.      the Card Guide.  You can either donate money or provide information 
  5.      on a card (that I do not already have in the guide of course).
  6.      You can outline below which one you wish to do:

  9.      ( ) $20.00 or more - Straight donation fee - I will mail you a disk with 
  10.                           the latest version of the Card Guide.

  12.      ( ) $10.00 + INFO - Fee for those who send info on one or more cards
  13.                          (use card info sheet in CARDG.DOC file).  You will
  14.                          be sent the latest version of Card Guide and save 
  15.                          $10.00 for your efforts.

  17.      ( ) INFO  -  For those who cannot afford to donate money but still
  18.                   wish to see Card Guide improved - you can donate info on 
  19.                   one or more cards (please use card info sheet in the 
  20.                   cardg.doc file).  For this price you I cannot send you the
  21.                   latest copy, but will include your information in the next 
  22.                   version which will be uploaded to several BBS's.

  47. The donation amount stated above is in Canadian Dollars and includes shipping
  48. within Canada.  Payments should be in the form of cash, cheque, postal or bank
  49. money order.  Those paying from outside Canada should add some for shipping 
  50. and must provide a type of fund that is redeemable at a Canadian Bank or 
  51. institution (ie. International money order cashable at a Canadian Bank) and 
  52. can pay the equivalant amount in US dollars.
